Preparing for your visit

To ensure the accuracy of your health records, all patients must register before their appointment. This applies whether you are a first-time or returning patient.

Required documents
Please bring the following documents to your appointment:

• Provincial health card (e.g. MCP, provincial health card or equivalent).
• Referral or requisition from your doctor, if applicable.
• Identification (for non-Canadian citizens, bring photo identification or passport, appropriate immigration forms or insurance documentation).

If your MCP coverage has expired, follow these steps to update your health card. You may need to complete a form on-site to request a new card.  

a. If your MCP coverage has expired within the past 12 months you can choose to renew your MCP using one of the following options:

• Prior to your visit
  • Complete the provincial MCP ‘Card Replacement/Information Update Form.’ You can find this form online at www.gov.nl.ca/mcp
  • Complete your MCP renewal by logging into your account at MyGovNL at www.mygovnl.ca.
    
• During your visit
  • Complete the MCP renewal form during registration or admission to validate your card and renew coverage. 
    If you complete the form, your MCP coverage will be renewed. If you choose not to complete the form, you will be registered as self-pay and incur any costs associated with services. 

b. If the MCP coverage has expired for 12 months or more:

• Complete ‘Application for Newfoundland and Labrador Health Care Coverage.’ You can find this form online at www.gov.nl.ca/mcp. 
• Complete the form during registration or admission to renew your MCP coverage.
• You will be registered as self-pay and receive a bill in the mail for services.
• Your file will be updated to MCP coverage once your MCP application is processed.

• Out-of-province patients: If you are visiting from another province, ensure you bring your provincial health card and any required forms.
• Non-Canadian citizens: Please bring photo identification or passport, your insurance details and relevant documentation for processing (i.e. passport).
• Workers’ compensation: If your visit is related to a workers’ compensation claim, please provide the claim number and injury details.
• Interpretations services: If you require interpretation services, please advise the clerk. We will do our best to meet your needs. 

Information collected during registration
For identification and record-keeping, you will be asked to provide:

• Full legal name and date of birth (as it appears on your health care)
• Health card number (e.g. MCP) and expiry date
• Contact details (address, phone, next of kin)
• Primary care and attending physician information
• Appointment notification preferences (phone, email or text)

Arrival time based on appointment letter:

• Check your appointment letter for specific arrival instructions.
• If it indicates the required arrival time (e.g. 30 minutes before the scheduled appointment), plan accordingly.

General arrival guidance:

• If your appointment letter does not specify an arrival time, please arrive at least 15 minutes before your scheduled appointment to allow for registration and to locate the appropriate department. 
• Account for travel and parking time when planning your visit, as parking availability may vary.
• Depending on the number of patients at registration when you arrive, there may be a wait time.
• If you require a support person during your visit, please plan for them to accompany you for the duration of your stay.
