If you’re a parent or legal guardian of a child under 12, you can request Proxy Access through MyHealthNL. Once approved, you’ll be able to access your child’s health information. Access will automatically expire when they turn 12.

1. Log into the proxy’s MyHealthNL account.
2. Click to open the Menu.
3. Scroll down and click Questionnaires/Screenings.
4. Select the “MyHealthNL Proxy Access Request Form”.
5. Enter the child’s information:
   • Full name
   • Date of birth
   • MCP #
   • Your relationship with the child
6. Select their age range.
7. Answer additional questions as prompted.
8. Upload any required supporting documentation.
9. Submit your request for review.

If you’re a parent or legal guardian of a youth between 12 and 15, you can request Proxy Access with the youth’s consent. Access will automatically expire when they turn 16, giving them full independence over their health information.

1. Log into the proxy’s MyHealthNL account.
2. Click to open the Menu.
3. Scroll down and click Questionnaires/Screenings.
4. Select the “MyHealthNL Proxy Access Request Form”.
5. Enter the youth’s information:
   • Full name
   • Date of birth
   • MCP #
   • Your relationship with the child
6. Select their age range.
7. Answer additional questions as prompted.
8. Upload any required supporting documentation, including the Youth Consent Form.
9. Submit your request and wait for approval.

If you’re 16 years of age or older, you can grant Proxy Access to someone you trust — like a family member, a parent, a partner, or caregiver. You stay in control at all times, with the ability to choose what they can see and do, and to update or remove access whenever you like.

1. Log into the patient’s MyHealthNL account.
2. Click to open the Menu.
3. In the top right corner, select your profile icon. Click “Manage Friends and Family”.
4. Under “Who can see my record” you can review who currently has access. To add a new proxy, click “Invite Friends or Family”.
5. Enter the required information for the person who want to invite.
6. Select the level of access you want to provide.

1. Review and agree to the Terms and Conditions.
2. Click “Send Invite”.

What Happens Next:
The invited person will receive an email invitation to accept Proxy Access. To confirm access, they will need to enter your date of birth.

If you are a legal representative or substitute decision maker for someone who does not have the capacity to make their own health care decisions, you can request Proxy Access to help manage their health information. You must already have a patient record with NL Health Services and be an active MyHealthNL user. NL Health Services may request additional documentation to verify your legal authority.

Yes, you can. MyHealthNL Proxy Access allows you to access the medical records of your family members and others you care for with their permission. You must be at least 16 years old to have Proxy Access to another person’s medical record.

Yes. To be a proxy, you must already have a patient record with NL Health Services and be an active MyHealthNL user.

Yes, you can change access levels at any time through your “Manage Friends and Family” settings.

Yes. You can remove access at any time through your “Manage Friends and Family” settings.

When a youth turns 16, Proxy Access automatically expires and they will have full control over their health information. If desired, the individual may reinstate Proxy Access by sending an invite from their MyHealthNL account.

When a child turns 12, Proxy Access automatically expires. If desired, you need to newly request access with their consent.

No, due to the sensitive nature of personal health information, each person must have their own MyHealthNL account. However, you may be a proxy for each other by requesting access if desired.
